We accept most major insurance plans, please call our office to inquire whether we participate in your particular plan, prior to your appointment, please check with your insurance company regarding referrals, co-payments, and any deductible required at the time of service. Our office staff will be prepared to discuss this information with you at your first visit. Please notify our office of any change in name, address, phone number or insurance information.

Co-payments / deductible are due at the time of service. For your convenience we accept cash, checks, Visa, Mastercard and Discover. Payment plans are available, check with the receptionist to see which option is best for you.
